C4: Advancing Safety Through Trust and Psychological Safety: Implementing Just Culture at Osler

William Osler Health System’s Just Culture initiative aimed to enhance safety culture and build psychological safety across the organization. Nursing leadership played a pivotal role in advancing this strategy, driving tailored training, multi-level engagement, and integrating Equity, Diversity, and Inclusion principles to foster shared accountability and a learning organization culture.

Krishna Jain, BMRSc, MRT(R), MBA William Osler Health System
Krishna Jain is a Quality Improvement Consultant with over 8 years of experience as a frontline Medical Radiation Technologist, which has included roles in teaching, technical expertise, and workflow management. As a QI consultant, she focuses on patient safety, High Reliability Organizations (HRO), and QI education through Osler’s QI Academy. Krishna has a strong interest in Human Factors and patient/family co-design. She holds an MBA from McMaster University, a Lean Six Sigma Green Belt certification, and a Bachelor’s in Medical Radiation Sciences from McMaster University/Mohawk College. Passionate about mentorship, fostering leadership and growth opportunities for others, Krishna is dedicated to driving positive change in healthcare.

Nimrah Kiyani, William Osler Health System
Nimrah is an internationally trained physician by background and a Quality Improvement Consultant by passion with over 9 years of experience both internationally and in Canada in quality improvement and patient safety. Her work in Osler focuses on enhancing the provision of safe and quality care to patients through quality improvement initiatives along with supporting the teams with their Accreditation preparedness activities. Nimrah has completed her MHSc in Health Administration from University of Toronto. Along with that she is also a Certified Professional Healthcare Quality as well as a Fellow of American Board of Quality Assurance and Utilization Review Physicians.
