Poster 7: A Centralized Team Model to Support Clinical Externs: Preparing Future New Graduate Nurses

In 2022, the SickKids’ Clinical Extern Program underwent significant restructuring, pivoting from a locally managed framework to a centralized model. Through centralization, current program strategies to support the Extern’s career pathway now enable the next cohort of inter-professional clinicians and nurses to transition through the organization’s workforce pipeline safely and smoothly.

Tiffany Yam, RN, BScN, MN, Professional Practice Coordinator (PPC) Clinical Extern Program, The Hospital for Sick Children

Tiffany Yam, RN, MN is a Professional Practice Coordinator (PPC) for the Clinical Extern Program and a Clinical Nurse Specialist (CSN) for Nursing Professional Practice at the Hospital for Sick Children. Tiffany’s role as a PPC practice mentor supports shaping and guiding unregulated care providers as nursing extenders, to release nursing time to care. As a CNS, she works to promote evidence-based nursing practice by designing and executing organizational-wide change management strategies.

Tiffany has a Health System Leadership and Administration Master of Nursing from the University of Toronto, and a Bachelor of Nursing Science from The University of Ottawa. Over the past 14 years, she has held clinical and leadership roles within the neonatal and pediatric acute care environment at The Ottawa General Hospital, The Civic Hospital, and The Hospital for Sick Children. She has been in varying roles of leadership throughout her career, including holding a Clinical Support Nurse position in General Pediatric Medicine and a Team Lead position to guide clinical and non-clinical staff to support the COVID-19 response strategy.

Tiffany is a passionate pediatric nurse that enjoys collaborative team building within complex adaptive environments and seeks to advance pediatric care through strengthening nursing acute care practice.
