Krishna Jain, BMRSc, MRT(R), MBA, William Osler Health System
Krishna Jain is a Quality Improvement Consultant with over 8 years of experience as a frontline Medical Radiation Technologist, which has included roles in teaching, technical expertise, and workflow management. As a QI consultant, she focuses on patient safety, High Reliability Organizations (HRO), and QI education through Osler’s QI Academy. Krishna has a strong interest in Human Factors and patient/family co-design. She holds an MBA from McMaster University, a Lean Six Sigma Green Belt certification, and a Bachelor’s in Medical Radiation Sciences from McMaster University/Mohawk College. Passionate about mentorship, fostering leadership and growth opportunities for others, Krishna is dedicated to driving positive change in healthcare.

Komal Mazhar is a dynamic and results-driven leader with over 14 years of proven success in strategic operations, quality improvement, patient safety, and project management. As the Director of Quality & Patient Safety at William Osler Health System, she leads transformative patient safety initiatives, and organization-wide quality improvement efforts, and ensures continuous accreditation readiness.
An accomplished healthcare professional, Komal’s contributions extend beyond her leadership role. She has authored publications and has been a speaker at industry conferences, reflecting her passion for driving excellence in healthcare quality and patient safety.
Her impressive educational background includes a Master of Science in Healthcare Quality from Queen’s University and a Master of Health Management from McMaster University. She also holds a strong foundation in Medical Radiation Sciences specialising in Nuclear Medicine Technology from the University of Toronto/The Michener Institute.
