B4: Synergies: Home and Community Care Clinical Practice Leaders Collaborate for Standardization in Clinical Practice & Care Partners

This session will demonstrate how collaboration between clinical leaders of several community Service Provider Organizations (SPOs) over the past several years has led to a unified approach to provide safe, consistent, equitable care, achieve optimal patient and system outcomes, and resulted in a healthy working relationship between the SPOs.

Michelle Pothier, MN, BN, RN, CHNC(C), CVAA(C), Clinical Consultant – Professional Practice Team, ParaMed Health Care

Michelle Pothier, BN, RN, MN, CCHN(C), CVAA(C) has worked in Home Health Nursing for the past 31 years. She currently works as a Clinical Practice Consultant for ParaMed Health Care from home in Prince Edward County, Ontario. Michelle promotes clinical best practice through education, policy and program development. She is currently focusing on policy and education involving wound care, infusion therapy, and documentation. Michelle has a passion for infusion therapy and is volunteering as part of the CVAA Guidelines revision group (GDG 2.0) for the 2024 Guidelines update. In her spare time, Michelle enjoys staying active by swimming, cycling, running and speedskating.

Sarah Burns, Director of Professional Practice and Integrated Care, CarePartners

Sarah Burns is the Director of Professional Practice and Integrated Care for CarePartners, who specializes in providing leadership in the home and community care sector. With over 25 years of experience in the home and community care sector, Sarah’s career has extended across clinical practice, quality assurance, implementation science, change leadership, education and learning strategies, guideline development and review, patient-centered improvement, integrated and transitional care, team building and coaching. She holds a Master of Science Degree in Community Health Nursing and a postgraduate certificate in education. She is certified by the Canadian Nurses Association in Community Health Nursing. She has completed research on the cultural competence of home health nurses and an advanced Clinical practice fellowship through the Registered Nurses Association of Ontario, which focused on outcomes and data evaluation pertaining to end-of-life care. She has participated on the Board of Directors for Canada’s National Vascular Access and Infusion Therapy Association, was the editor-chief for the CVAA’s journal, participated in writing the Canadian Guideline on vascular access and infusion therapy, taught as an adjunct professor for Conestoga College and McMaster University Nursing Program, participated as an expert at advisory board meetings, and has spoken internationally. She received the Toronto Sick Kids Award of Excellence for Community Care in 2022. Her passions are providing high-quality, evidence-informed care to patients in the home and community care sector, supporting vascular access and infusion therapy and pediatric care to patients, and transitioning patients to the home and community space.
