B1: Advancing Nursing Practice and Academic Excellence through Clinical Scholarship: TheAcademic Practice Partnership Model

The Academic Practice Partnership (APP) model, an innovative and strategic partnership between a large healthcare organization and an academic institution was established in 2019 to develop a strong learning culture in the clinical setting and provide better support to students’ transition to independent practice. Clinical units that implemented the APP has resulted in better clinical staff engagement and higher patient experience scores, lower falls and pressure injury rates, and a lower length of stay when compared to other medicine units at Osler.

Tiziana Rivera, BScN, MN, EdD, William Osler Health System
Tiziana Rivera is a visionary nurse executive who has provided strategic change leadership in her clinical, professional practice, administrative and governance roles. As a values based, solution oriented professional, she has mentored high performing teams to support the delivery of safe, evidenced informed, high quality care across the continuum of care. She has extensive experience and expertise in the development and implementation of innovative models of care within the context of interprofessional practice. Tiziana’s leadership encompasses Professional Practice, Academic Partnerships, Research, Quality, Patient Safety, legal & Risk
Tiziana has worked closely with the Registered Nurses’ Association of Ontario to support the international adoption of best practices and supported two Ontario hospitals in their journey towards achieving Best Practice Spotlight Organization status. She has served on several Boards and is currently teaching in the undergraduate nursing program at Toronto Metropolitan University and holds an Adjunct Lecturer appointment at the Lawrence S. Bloomberg Faculty of Nursing, University of Toronto and an Adjunct Professor position at Humber College.

Charissa Cordon, BScN, MN, EdD, William Osler Health System
Dr. Charissa Cordon has 2 decades of experience in healthcare, as a nurse, educator, and leader. She currently holds the role of Associate Vice President, Practice and Academics at William Osler Health System. She has a wealth of experience as a transformational change leader in nursing, and professional practice and has led many initiatives at the local, provincial, and national levels.

Prior to her role at Osler, Charissa has held senior leadership roles as Director of Professional Practice and Clinical Resource Team at Trillium Health Partners, and as Chief Nursing Information Officer and Chief of Interprofessional Practice, Oncology at Hamilton Health Sciences, where she led many initiatives, including supporting Internationally Educated Nurses’ integration in the workplace. Charissa is interested in exploring models of care, oncology nursing practice and educational initiatives, and optimizing nursing roles in the workplace. She has published her work in these domains. She currently holds an adjunct faculty appointment with the Lawrence Bloomberg School of Nursing, University of Toronto, and she is also currently the Vice-President elect as a Board-member for the Canadian Association of Nurses in Oncology.
