Agnes Oriade, BScN, MN, SE Health
Agnes Oriade is a Nurse Manager at SE Health, overseeing four community nursing clinics in the York and Simcoe areas. With nearly 15 years of nursing experience, she has focused on transforming care delivery and advancing nursing practice. Previously, Agnes was a Telemedicine Nurse Facilitator, where she helped implement a community-based care model linking primary, acute, and community services to address complex patient needs. She then transitioned to the role of a Clinical Practice Coach, providing leadership to nursing teams through coaching, mentoring, and quality improvement initiatives. Agnes is passionate about collaborating with nurse leaders to promote nursing excellence and drive continuous improvements in patient care, with a strong commitment to innovation and quality in nursing practice.

Ade Oyemade, RN, BScN, MN, SE Health
Ade Oyemade is the Regional Director for Central Toronto, Central West, and Mississauga Halton at SE Health, overseeing large-scale operations and working with leaders to ensure optimal care for clients. Previously, she served as Director for Nursing, Rehab, and PSW, managing clinical operations and quality outcomes for homecare clients. Ade was also Executive Director at Hillcrest Reactivation Center, where she led a healthcare team, implemented new protocols, and streamlined processes to improve client outcomes and operational efficiency. With 25 years of experience, Ade is passionate about leading clinical operations, delivering patient-centered care, and fostering a collaborative, continuous improvement culture within teams.
