A4: Supporting Evaluation Capacity: Co-creation of a Logic Model in the Transition Mentorship Program

The Transition Mentorship Program brings together mentor roles from provincial health human resource programs. In collaboration with the University of Waterloo, a multi-faceted approach was used to co-create a program logic model with integrated capacity building that aimed to break down silos, standardize processes, and ground the program in evidence.

Kimberly Lawrence, RN, BScN, MN, Sunnybrook Health Sciences Centre
Kimberly Lawrence is an Advanced Practice Nurse working with Interprofessional Practice at Sunnybrook Health Sciences Centre. Kimberly’s nursing experience is focused on trauma and intensive care, where she is passionate about person-centred, quality care practices and integrated care models. She holds a Masters of Nursing in Leadership in Healthcare Policy and Education, as well as certificates in Quality & Patient Safety and Inclusion, Diversity, Equity, & Allyship. Kimberly is providing leadership in health human resources, nursing strategy, mentorship, and novice nurse transitions. Most recently, Kimberly has collaborated in advancing the Multihospital Critical Care Resource Team and Transition Mentorship Program.

Tracey DasGupta, RN, MN, Sunnybrook Health Sciences Centre
Tracey DasGupta, RN, MN is a nurse leader and Director of Interprofessional Practice at Sunnybrook Health Sciences Centre, and adjunct lecturer at the Lawrence Bloomberg Faculty of Nursing. With over 30 years of experience, she has led initiatives that promote collaboration, streamline patient care, and ensure equitable healthcare access. Tracey is passionate about fostering inclusive, person centred environments and empowering healthcare teams. A strong advocate for nursing leadership, she works to enhance the role of nurses in driving change and improving outcomes. Her efforts emphasize the importance of integrating diverse perspectives to address healthcare disparities and promote sustainable, equitable practices.
