In April 2021, with the help of champion nurses, SE Health co-designed and implemented an innovative MySE Life Application, a first of its kind, in-house built mobile application experience that digitized and consolidated direct care workers calendar and caseload with real-time access to client information and schedules. This tool has empowered nurses, through data, to make informed decisions surrounding client care in the home care environment.
Candace Moore, Digital Product Lead, SE Health’s H.O.P.E Model of Care
Candace Moore is SE Health’s first Digital Product Lead and acts as a binding agent between operations, clinical and technology, prioritizing employee experience and SE Health’s H.O.P.E Model of Care at the center of the design process. Candace works closely with direct care providers co-designing the MySE Life App, which enables nurses, personal support staff and therapists with real-time access to client and visit information empowering staff to be accountable and autonomous with reporting, continuity, scheduling and communication changes to the office all in a consolidated customized mobile view.
